In the first quarter of 2026, Long Term Care Management Services, LLC, John McNeill and Ronald McNeill owned Liberty Commons Nursing and Rehabilitation Center, according to Centers for Medicare and Medicaid Services (CMS).
The facility is designated as for-profit.
Throughout this period, Liberty Commons Nursing and Rehabilitation Center earned an overall CMS rating of one, falling short of the state average CMS rating of 2.9 for North Carolina nursing homes. The CMS assigns these ratings after reviewing several factors, including results from health inspections, staffing numbers, and the overall quality of resident care.

Liberty Commons Nursing and Rehabilitation Center incurred $135,234 in five fines and received five penalties during the same quarter of 2026.
Altogether, Columbus County, North Carolina, reported three for-profit nursing homes.
Data from the U.S. Census Bureau show the number of people in older age groups increasing, while the under 18 population declines. The nation’s 65-and-older group grew by 13% between 2020 and 2024, as those younger than 18 dropped by 1.7%. Over the past 100 years, the U.S. older adult population has greatly expanded, growing from 4.9 million in 1920 to over 61 million by 2024.
A 2021 study estimates almost half of individuals in this group—approximately 28 million—may require long-term assistance and support. More than one-third are projected to eventually need nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an educational organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
