For that period, the center earned an overall CMS rating of 1, compared with the state average of 2.9 for nursing homes in North Carolina. CMS assigns ratings based on key measures such as health inspection performance, staffing levels and resident care quality.
The facility averaged care for 69.5 residents per day in the first quarter of 2026, with 127 licensed beds. It is not hospital-based.
Premier Living and Rehab Center incurred a total of $381,817 in two fines and faced four penalties as of the first quarter of 2026.
Altogether, Columbus County had 3 for-profit nursing homes.
According to U.S. Census Bureau data, the nation’s population of older adults is increasing while the number of children declines. Between 2020 and 2024, Americans age 65 or older grew by 13%, while those under 18 decreased by 1.7%. The growth of the older adult population has accelerated over the past 100 years, from 4.9 million people in 1920 to over 61 million in 2024.
A study published in 2021 estimates that nearly half of Americans in this age bracket, approximately 28 million, are expected to need long-term services and supports. The research also suggests more than one-third could eventually require care at a nursing facility.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
